CRITICAL CARE · OCTOBER 29, 2026
Adding chlorhexidine to standardized mechanical oral care did not reduce ventilator-associated pneumonia.
Across 1,035 ventilated adults and 10,024 ventilator-days, twice-daily 0.12% chlorhexidine produced a VAP rate of 18.8 versus 16.9 per 1,000 ventilator-days without chlorhexidine.
Preserve toothbrushing, moisturization, suctioning, and secretion management, but do not assume routine chlorhexidine adds benefit. The nonsignificant result does not prove equivalence, and the confidence interval permits modest benefit or harm.
